OVER 75 HEALTH ASSESSMENTS
Each year North East Family Medicine invites our over 75 year old patients to attend the practice to have their Health Assessment completed. There is no cost for this service as it is covered by Medicare.
WHAT IS AN OVER 75 HEALTH ASSESSMENT?
An Over 75 Health Assessment is a comprehensive assessment of your health and your physical, psychological and social function. It helps to assist in the early identification of care needs.
WHO IS ELIGIBLE TO RECEIVE A HEALTH ASSESSMENT?
You are eligible if you are aged 75 years or over and living in a community setting. You must be a regular patient of the surgery. Patients are only eligible to have one health assessment completed each year.
WHAT HAPPENS DURING A HEALTH ASSESSMENT?
The health assessment will take approximately 1 hour to complete.
You will spend between 20-40 minutes with the Practice Nurse who will perform some simple tests and ask you some questions about your medical history.

Following this you will speak with your doctor for 15-20 minutes.
A detailed record of this assessment with the outcomes and recommendations is recorded in the patient file and the patient is given a copy to keep.
45-49 YEAR OLD HEALTH CHECKS
As we get older, many of us become more vulnerable to illness. The federal government introduced a new health check in 2008. This health check is for everyone aged between 45 and 49 who may be at risk of developing a chronic health issue like diabetes or heart disease. The aim of the health check is to help find, prevent or lessen the effect of disease. This health check will give us the opportunity to look at your lifestyle and medical/family history to assess your risk of developing a chronic disease. There is no cost for this service as it is covered by Medicare. Please note: You are only eligible for one 45-49 year old health check.
